Making a referral

Both, Agency Referrals and Sel-referrals are made by phoning our office on 01274 - 495834.
Risk and Needs Assessments are done over the phone. We will need details of:
- The agency doing the referral, workers name and contact number
- The womans name, date of birth and NI number
- Age and sex of any children
- Details of her income
- Her reason for referral
- If there are any drug, alcohol or mental health issues
- If she has been convicted of Schedule 1 or arson offences
- If she has any mobility problems
- If she is willing to work with a support plan
